WHAT IS IT?
PLAYERSPACE allows you to create surveys and questionnaires for parents, coaches, and athletes to complete.
I WOULD LIKE TO...
CREATE A GENERAL USE SURVEY
1. Select the program from the dropdown
2. Click the navigation icon in the top-left corner

3. Click Create Survey

4. Click the General Use tab on the left side of the screen
5. To create a new survey, enter the number of questions you will have in the survey
Note: To import a survey from a previous program, you can do so on the right side of the screen under Option 2

6. Use the dropdown to select Yes or No if you will be collecting background check info
7. Use the dropdown to select Yes or No if you will be collecting uniform size info
8. Click the Continue button

9. Enter a name for your survey
10. Enter a Confirmation Page Message

11. Type your question in the Question Text box
12. Enter a General Question Heading or a Disclaimer if you would like (Optional)
Note: General Question Headings or Disclaimers will be displayed above the question in the survey.
13. Complete the Question Settings
14. Select the question type from the dropdown
Note: For multiple choice questions, enter the choices in the Multiple Choice list box separated by commas
15. When finished, click the Update Questionnaire button

EDIT SURVEY NAMES
1. Select the program from the dropdown
2. Click the navigation icon in the top-left corner

3. Click Survey Names

4. To change the display name of a general use survey, type the new name in the box under the Label column
5. If you would like to hide the survey , switch the toggle to Yes under Hide Path?
6. Click the Update Path Settings button to save changes
